Your clinical history, provider correspondence, treatment protocols, and test results are held centrally by Zenify. Every provider, in your case, works from the same picture.
For members accessing care through TRICARE or private insurance, PHM includes documentation support, pre-authorisation management, and coordination with the insurer across multiple visits and providers.
PHM does not close on a schedule. It remains active for as long as your case requires active management. Scope is reviewed at regular intervals and adjusted accordingly.

There is no fixed monthly rate because no two PHM cases carry the same workload.
Pricing is established after an initial case assessment, structured as a monthly retainer with a defined scope, and reviewed every three to six months. If complexity increases or decreases, the scope and price adjust accordingly.
